At Colusa Medical Center, our Social Services team is dedicated to helping patients and families navigate the challenges that can accompany illness, hospitalization, and recovery. We understand that healthcare extends beyond medical treatment, which is why our experienced social workers provide compassionate guidance, education, and access to resources that support each patient’s overall well-being.

Working closely with physicians, nurses, therapists, case managers, and community organizations, our social workers help patients and their families identify available resources, coordinate services, and develop personalized plans for continued care. Whether planning for a safe discharge, connecting with community support programs, or assisting with long-term care needs, our team is committed to helping patients access the services they need.

Our support doesn’t end when a patient leaves the hospital. Through collaboration with our Home Health and Hospice programs, we continue to provide guidance and connect patients and families with resources that promote independence, comfort, and quality of life throughout their healthcare journey.
