Requesting Your Medical Records

Patients have the right to access their medical records in accordance with applicable federal and California laws.

To request a copy of your medical records, you may be asked to complete an Authorization for Release of Health Information form and provide valid photo identification. Additional verification may be required to protect the privacy and security of your health information.
