Colusa Medical Center’s Swing Bed Program provides short-term skilled nursing and rehabilitation services for patients who no longer require acute hospital care but are not yet ready to safely return home. Our program offers a comfortable, supportive environment where patients can continue their recovery while receiving personalized medical care and rehabilitation services designed to restore strength, independence, and confidence.
Whether recovering from surgery, illness, injury, or hospitalization, our multidisciplinary team works closely with each patient to develop an individualized care plan focused on achieving the highest possible level of function. Our goal is to help patients safely transition home while improving their overall quality of life.
Patients admitted to the Swing Bed Program receive comprehensive care from experienced physicians, registered nurses, rehabilitation therapists, case managers, and other healthcare professionals. Through coordinated treatment and daily therapy, we help patients regain mobility, improve independence, and prepare for a successful return home.
Our Swing Bed Program is Medicare-certified, and we work with Medicare and many private insurance providers to help patients understand their coverage and coordinate benefits whenever possible.

Many patients complete the program within approximately three weeks, although the length of stay is individualized based on each patient’s medical condition, rehabilitation goals, and physician recommendations.
Referrals may be made by physicians, case managers, discharge planners, healthcare providers, caregivers, patients, or family members.
