Partner With Your Care Team

You are an important member of your healthcare team. We encourage you to ask questions, share concerns, and participate in decisions about your care.

Throughout your visit, you will interact with physicians, nurses, pharmacists, therapists, laboratory professionals, imaging specialists, nutrition services, environmental services, and many other dedicated healthcare professionals. Your nurse will often be your primary point of contact and will help coordinate your care during your stay.

If you ever have questions about your diagnosis, medications, procedures, or treatment plan, please speak up. Our team is here to listen, answer your questions, and help you feel informed and confident throughout your healthcare experience.

Registration & Check-In

All patients should begin their visit by checking in at the Registration Desk located in the hospital’s main lobby.

To help make your registration process as efficient as possible, please bring:

  • A government-issued photo ID

  • Your insurance card

  • Physician orders, referrals, or laboratory requisitions, if applicable

  • Workers’ Compensation information, if applicable

  • Your Advance Healthcare Directive, if you have one

  • A method of payment for any applicable copays, deductibles, or non-covered services

If you require special accommodations, including language interpretation, hearing or vision assistance, wheelchair assistance, or other accessibility services, please notify our registration staff when you arrive. We are committed to ensuring every patient has equal access to quality healthcare.

Communication

Open communication is one of the most important parts of quality healthcare. We encourage you to discuss your concerns, ask questions, and involve your family or designated support person in your care whenever appropriate.

If at any time you do not understand your diagnosis, treatment plan, medications, or discharge instructions, please let a member of your healthcare team know. We are committed to providing information in a way that is clear, understandable, and respectful of your individual needs.

Medications

For your safety, please bring an up-to-date list of all medications you currently take, including:

  • Prescription medications

  • Over-the-counter medications

  • Vitamins

  • Herbal supplements

  • Other dietary supplements

Knowing your current medications helps our healthcare providers prevent medication interactions and provide the safest possible care.

If you have questions about any medication prescribed or administered during your visit, please ask your physician, nurse, or pharmacist.

Patient Identification

To help ensure your safety, you will receive a patient identification wristband during registration.

Please verify that your name and date of birth are correct. Members of your healthcare team will check your identification before administering medications, collecting laboratory specimens, performing imaging studies, or providing treatments and procedures. These safety checks help us provide the right care to the right patient every time.

Personal Belongings

For your comfort, you may wish to bring personal hygiene items such as:

  • Toothbrush and toothpaste

  • Shampoo and conditioner

  • Hairbrush or comb

  • Other personal care items

The hospital provides gowns, linens, towels, and basic personal care supplies during your stay.

To protect your valuables, we recommend leaving jewelry, large amounts of cash, expensive electronics, and other valuable personal belongings at home whenever possible. Colusa Medical Center cannot assume responsibility for lost, stolen, or damaged personal property.

Advance Healthcare Directives

An Advance Healthcare Directive is a legal document that allows you to communicate your healthcare wishes if you become unable to make medical decisions for yourself.

If you have completed an Advance Healthcare Directive or designated a healthcare decision-maker, please bring a copy with you so it can be included in your medical record.

If you have questions about Advance Healthcare Directives, our staff can provide additional information during your visit.

Immunizations

Please let your healthcare provider know if you have recently received vaccinations, including influenza, pneumococcal, COVID-19, or other recommended immunizations. This information may assist your healthcare team in providing the most appropriate care.
